Meaning:Om. Lead me from the unreal (asat) to the real (sat). Lead me from darkness (tamas) to light (jyoti). Lead me from death (mrityu) to immortality (amrita). Om, peace, peace, peace. The Upanishad itself explains that 'the unreal' and 'darkness' both stand for death, and 'the real' and 'light' for immortality, so all three lines ask to be made immortal.

अर्थ:ॐ। मुझे असत्य से सत्य की ओर ले चलो। मुझे अंधकार से प्रकाश की ओर ले चलो। मुझे मृत्यु से अमरता की ओर ले चलो। ॐ शांति, शांति, शांति। उपनिषद स्वयं समझाता है कि 'असत्' और 'अंधकार' दोनों मृत्यु के प्रतीक हैं और 'सत्' और 'ज्योति' अमरता के, इसलिए तीनों पंक्तियाँ अमरता की ही प्रार्थना हैं।

Which version we use

The three lines are identical in every source. The Upanishad (1.3.28) writes the third line with sandhi as 'मृत्योर्माऽमृतं गमय' (mrityor ma amritam, the 'a' of amritam merging); we keep that spelling, which Sanskrit Documents (in the Upanishad text) and English Wikipedia give; Sanskrit Wikisource and Hindi Wikipedia write the same sandhi without the avagraha sign ('मृत्योर्मामृतं'), and some popular sites such as Green Message separate the words as 'मृत्योर्मा अमृतं गमय'. The pronunciation is the same. The Upanishad ends the lines with 'iti' (thus) and has no opening Om and no 'Om Shantih Shantih Shantih'; these are added in recitation. 'Sadgamaya' and 'Jyotirgamaya' are also written as two words, 'sad gamaya' and 'jyotir gamaya'.
